ADVISORY

Before filling-up this form please prepare the following:
For Grade 7 (ESC Applicants) These documents must be submitted in person at the Guidance and Testing Office
1. Report Card - Grade 6 (jpg format)2. PSA Birth Certificate (jpg format)3. Mother's or Father's Latest Income Tax Return / proof income at least (3)months (jpg format)4. Proof of Income / Payslip (if family have unstable /no fix income, e.g. - sari-sari store owner or       online seller , provide Barangay Certificate of No Fixed Income. (jpg format)5. Baptismal Certificate6. Parents marriage certificate (if parents are not yet married please provide PSA mother's birth       certificate instead) (jpg format)7. Good moral certificate (jpg format)8. Barangay Certificate of residency (either of the parents) (jpg format)

For NEW Students1. Report Card - with 2nd/3rd quarter grade (jpg format or pdf.)    Other Documents can be submitted on a later date online or in person.
